The attacks, which reportedly occurred between Monday and Tuesday, affected communities in the Ugboju District of Otukpo LGA and the Moon Council Ward of Kwande LGA.
The deadliest incident occurred in the early hours of Tuesday when armed attackers reportedly invaded Efeyi community in Ugboju, killing at least 14 residents and leaving several others with gunshot injuries.
Two additional deaths were reported in separate attacks in Otukpo and Kwande, bringing the reported death toll from the latest wave of violence to at least 16.
The casualty figures were based on accounts from local authorities and community sources and could change as search efforts continue and more information emerges.
Residents said the attackers entered Efeyi community at about 4 a.m. on Tuesday, opening fire and causing residents to flee their homes.
A local source described the situation as devastating and alleged that security personnel did not arrive in the community for several hours after the attack began.
The source said some residents remained unaccounted for while efforts were underway to locate missing persons and assist the injured.
The Chairman of Otukpo Local Government Area, Maxwell Ogiri, confirmed that 14 bodies had been recovered following the attack.
Ogiri said several survivors sustained injuries and were receiving medical attention.
He described the incident as an unprovoked attack and said fear had spread to neighbouring communities, with some residents reportedly leaving their homes over concerns about further violence.
The reported delay in the arrival of security personnel has renewed concerns about emergency response capacity in vulnerable rural communities.
Residents alleged that security formations were located within the wider Ugboju area but did not reach Efeyi during the early stages of the attack.

Before the Efeyi attack, another resident was reportedly killed on Monday while working on a farm in Ipom community, also located in the Ugboju District of Otukpo LGA.
The reported killing has increased concerns among farming households, particularly as the attacks are occurring during the rainy season when many residents depend on regular access to their farms.
Repeated violence has forced some farmers to reduce the time they spend on their land or avoid farms considered vulnerable.
For many rural families, the loss of access to farmland can quickly affect household income, food supplies and the ability to meet basic needs.
In a separate incident, armed attackers reportedly invaded Abaji community in Moon Council Ward, Kwande LGA, where farmers were working on a Bambara nut farm.
A woman who sustained gunshot injuries during the attack was later reported dead.
Survivors said the attackers opened fire, forcing farmers to flee and abandon their farming tools.
The assailants were also reported to have taken two motorcycles belonging to residents who escaped from the area.

On July 13, armed assailants attacked Otukpo-Nobi community, with police confirming eight deaths and five injuries, although local accounts gave a higher casualty figure.
A separate attack on Ondo community in Ugboju was later confirmed by the Benue State Police Command, which reported that the community's youth leader was killed and several other residents were injured.
The repeated attacks have increased anxiety among residents of Otukpo and neighbouring communities, with many families expressing concerns about their safety and the possibility of further displacement.
